Overhauling Community Proposal

This commit makes considerable changes to the organization and content
of the existing community proposal, including a new section
that sets out to describe the problem we're trying to solve
in an abstract sense, so that readers can better understand
the intent and challenges we face in creating the Swift Package Manager.
