[index] Subsume CallRefIndexSymbol into IndexSymbol

Eventually we should just have one IndexSymbol, with all the
information. Once we kill FuncDeclIndexSymbol and can get rid of the
enum,  we will regain most of the bytes we lost by inlining receiverUSR.
